In a 4-week double-blind, placebo-controlled trial in 50 middle-aged adults with obesity, urolithin A 1000 mg/day did NOT improve the primary outcome (brachial artery flow-mediated dilation) or any secondary peripheral endothelial measure. A secondary brain measure (fNIRS task-evoked prefrontal oxygenation response during a working-memory task) increased versus placebo, with no change in cognitive task performance. Null primary; the brain signal is a secondary surrogate finding needing replication.
